What to Bring to Your First Appointment

To ensure a smooth and efficient first visit, please bring the following items with you to your appointment.

Photo ID

A valid government-issued photo identification (driver's license, passport, etc.)

Insurance Card

Your current insurance card. Copayments are due at the time of service.

List of Current Medications

Include all prescription medications, over-the-counter drugs, and supplements you are currently taking.

Imaging Disc

A DISC of previous images (X-ray, CT, MRI, etc.) — Must have the disc, not just the report.

Tips for Your First Visit

Arrive 15 minutes early to complete any remaining paperwork
Wear comfortable, loose-fitting clothing for your examination
Bring a list of questions you'd like to ask Dr. Burton
If possible, bring a family member or friend for support
Have your referring physician's information available
